I still use my married name, even though I’ve been divorced 25 years. I’d now like to apply for a passport ¬– I’ve never had one before – but the only documentation I possess is my birth certificate. I understand I need a copy of my marriage certificate or divorce certificate. How would I go about getting these?
The only documentation you actually need is your birth certificate and something to verify that the name you want on your passport is the one you actually use. The passport agency will accept a signed statement made by an older relative, or by a “professional” (such as your doctor) as long as they have known you in both your maiden and married names.
